REGISTRATION OF TRAINEES
New trainees must submit the registration form to the Credentialing Committee prior to beginning training. Please note that Training SIte MUST be CCMG Accredited in the trainee's specialty in order to qualify for training.
Please note, it is important that the CCMG management office be informed of any changes to the contact information provided. Failures to do so may result in processing delays if additional information is required to complete your credentials review.
WITHDRAWAL OR LEAVE OF ABSENCE
Trainees who are withdrawing from the program or who are taking a leave of absence from their training program are asked to submit a Withdrawal/LOA Trainee Form to the Credential Committee.
Trainees must maintain an accurate log book of their training and rotations.
The Training Director must review all entries in the trainee's logbooks, verify that the trainee had a major involvement in the cases listed and submit a signed letter to the CCMG electronically, attesting that he/she has reviewed the logbooks and signed off on them. The CCMG office must receive this letter by August 1 of the year that your credentialling application is submitted.
EDUCATION LOGBOOK SUMMARY SHEET
Education Logbook Summary will help standardize information the Credential Review Committee would like to see and should ensure that required information is submitted from all candidates. This does not replace the more detailed logbook but is meant to supplement it. (Trainees may still choose to also submit a complete a more detailed education logbook)

Sample faculty/rotation evaluation forms - In-Training Evaluation Report (ITER) and Final In-Training Evaluation Report (FITER) - are slightly modified versions of forms provided by the McGill University residency training program.
FINAL IN-TRAINING EVALUATION REPORT
The Final In-Training Evaluation Report (FITER) format was revised in 2013. The FITER is now both specialty and guideline version specific. The guideline version is the revision date indicated on your guidelines; the key competencies listed in the guidelines should match those listed on the FITER.
