The Local CCMG Training Program Director will have provided the names and emails of key individuals involved in the CCMG training program. The named individuals will receive invitations via email to complete online forms.
The accreditation application will be comprised of the following online forms:
- Application Form for the Local CCMG Training Centre Committee
- Application Form for the CCMG Training Centre’s Supervisory Committee*
- Application Form for the Discipline Training Program*
- Survey of Trainee Experience (current, past trainees)
*One for each of the four specialty training programs.
